BANK STATEMENT – Showing you have sufficient funds for your stay in Indonesia, last 1 months period (including name, date of period, and balance account)

STATEMENT LATTER GUARANTOR – Proof of guarantee from the guarantor and Statement from government agency/private institution, or statement from the applicant which explains that the foreigner will conduct medical treatment in the Indonesian Territory
